Remote Nurse Practitioner / Physician Assistant

(REMOTE - You can live anywhere in the state of AZ as long as you are licensed in AZ.) This is a unique opportunity to deliver high-quality, patient-centered care directly in the home—focusing on Chronic Kidney Disease and primary care needs. If you’re passionate about building relationships, improving outcomes, and working with autonomy while still being part of a collaborative team, this role is for you.
